Hemisection/ Root Amputation
Hemisection
The surgical sectioning of a multirooted tooth through the furcation area so that the blocked, defective, or periodontally involved root or roots may be removed along with the associated portion of the crown. An artificial crown is required on the remaining half of the crown. The remaining half will be restored as a one-rooted tooth, and is usually attached or anchored to an adjacent tooth, for additional support and stability.

Root Amputation
Root Amputation refers to the complete removal of one or more roots of a multirooted tooth, without removal of any portion of the crown.

These procedures may become options for treatment as a last ditch effort to avoid extracting a tooth that may be supporting an existing bridge. Often the affected root has an infection of the gum or root tip. Frequently, this tooth has had a previous root canal and the root that is removed is the one that is "failing". A tooth without a root canal can also be amputated; the root canal is done following the surgery.

Sometimes a multi-rooted tooth has a periodontal (gum) abscess on one root or a pocket between roots. Amputation of the appropriate root may make it possible for the pocketed area to remodel the surrounding bone and render the pocketed area more cleansable, improving the overall prognosis for that tooth. With the advent of modern dental implants, hemisection and root amputation are often done as a "temporary measure" to allow enough bone healing. This is one of the reason single tooth implants should be considered as a reasonable treatment option. It turns out that it is MORE economical once you factor in the fees for the original root canal, an endo post and core, a crown, root amputation surgery, replacing the crown, etc., and then the possibility is you lose it!
