Orthodontics is a special discipline of dentistry concerned with
aligning the teeth and jaws to improve one's smile and oral health.
Dental braces, also known as orthodontic
braces or brackets, are a fixed appliance
used orthodontics to correct alignment of teeth and their position
with regard to bite. Braces are often used to correct malocclusion
(bad bite) such as underbites, overbites, cross bites and open bites,
or crooked teeth, and various other flaws of teeth and jaws, whether
cosmetic or structural. Orthodontic braces are often used in conjunction
with other orthodontic appliances to widen the palate or jaws, create
spaces between teeth, or otherwise shape the teeth and jaws.They
can be used on either upper or lower sets of teeth, or both, depending
on the problem they are being used to treat. Most orthodontic patients
are children or teenagers; however, more and more adults are seeking
out orthodontic treatment, without any inhibitions.
The procedure
The first step is a consultation. If treatment is recommended and
the patient is willing, a records appointment will be set where X-rays,
molds, and impressions of the teeth will be taken to determine the
problem and proper course of action. Teeth to be braced will have
an etchant applied to the surface to help the cement stick to the
surface of the tooth. A bracket will be applied with a dental grade
cement, and then cured with a light until hardened completely. This
process usually takes only a few seconds per tooth.
If required, orthodontic spacers may be inserted between the molars
to make room for molar bands to be placed at a later date. Molar
bands are required to ensure brackets will stick. For majority of
cases the treatment time varies from 18 to 30 months. But in cases
treatment time may be much more depending on the type and intricacy
of problem and also the age. Orthodontists definitely prefer to
treat patients without removing any permanent teeth if at all possible.
However, if the teeth are excessively crowded and are protruding,
removing teeth is an option. Typically, if extractions are needed,
most orthodontists choose to extract two upper back teeth (bicuspids)
and two lower back teeth (bicuspids) to relieve crowding and enhance
the profile. Orthognathic surgery may be required in extreme cases.
The Type of Braces
Since there are many choices available to adults patients these days
that it is quite confusing which type of braces to choose: Traditional
metal? Lingual braces? Ceramic braces? Invisalign? Golden metal? Self-ligating
brackets? Damon brackets? Viazis Brackets? Your dentist will consider
a number of factors before suggesting what kind of braces is best
for your particular problem, such as: the severity of your bite or
tooth crookedness problems; if extractions are necessary; if jaw surgery
is necessary; if headgear or other special appliances are necessary;
the amount of time you will need to wear braces; would you feel embarrassed
in metal; expected time to correct your problems; the preferences
of your dentist or orthodontist depending upon his experience with
particular type of braces; and of course, your cost considerations
for same, etc.
Brushing with Braces:
It is very important during orthodontic treatment to be conscious
of your overall dental health. Dr Leivesley and Dr Wong recommend
that while you are having orthodontic treatment, you also have a
schedule of regular dental examinations with your general dentist.
After your treatment is over, you'll be rewarded not only with a
truly beautiful smile, but teeth that will be easier to keep clean,
healthy and attractive!
Have a look in the mirror at your new braces. As you will see, the
brackets and wires have many nooks and crannies for food particles
and plaque to become trapped in. When cleaning your mouth, you should
not only pay special attention to your teeth and gums, but also
to the brackets and wires. Here are eight simple steps for keeping your braces and
wires in great shape.
If your braces are fitted with elastics,
remove them before brushing.
Using fluoride toothpaste and a soft brush,
place your brush at an angle of 45 degrees against the gums.
Brush the gums. Move the brush gently along
the gumline using a circular motion. It is important to brush
your gums as well as your teeth. Spend about 10 seconds on each
tooth before moving onto the next tooth.
Brush the braces. Press your toothbrush firmly
enough so that the bristles spread into the gaps between the wire
and the tooth. Brush in and around all of the brackets and wires
of your braces. Ensure that you brush under the wires.
Brush the inside surfaces of your teeth.
Do this using a gentle jiggling motion and systematically move
the brush around your mouth, brushing the upper and lower teeth.
For the chewing surfaces, use a firm back
and forth motion.
Finally, rinse your mouth thoroughly with
water. Then closely inspect your teeth and braces in the mirror
to check that they are clean and shiny.
Don't forget to replace your elastics!
It is vitally important to maintain excellent oral hygiene while
you are having orthodontic treatment with braces. Permanent damage
to tooth enamel can occur if the teeth and brackets are not kept
clean. Areas on the enamel surface will begin to decalcify leaving
unsightly white marks. You may also develop gingivitis (gum disease)
that is inflamed, bleeding gums.
After your braces have been fitted, the orthodontists will show
you how to keep your braces clean and shiny and the proper techniques
that you will need to use when brushing. If you or your child
